Job Description
Elitecare Medical Staffing is seeking a compassionate and experienced Travel Registered Nurse (RN) for the Care Manager role in Home Health in Clearlake, CA. This position involves providing high-quality nursing care to patients in their homes and requires a strong commitment to patient-centered care.
Key Responsibilities
- Perform comprehensive assessments of patients in their home environment to develop individualized care plans.
- Collaborate with multidisciplinary teams to coordinate patient care and services.
- Monitor patient progress and adjust care plans as necessary to meet health goals.
- Educate patients and families about managing their health conditions and medications.
- Document patient interactions, assessments, and care plans accurately and timely.
- Travel to patients’ homes as required; mileage reimbursement at government rates.
Requirements
- Current Registered Nurse (RN) license in California.
- Minimum of 2 years of experience in Home Health preferred.
- Clean driving record is required.
- Basic Life Support (BLS) certification.

- Work Schedule: Day shift, 40 hours per week.
