Position Title: Travel -Physical Therapist - Home Health

Salary Range: $50 - $60 per hour
Job Summary:
We are looking for a dedicated and compassionate Physical Therapist to join our Home Health team in Portland, Oregon The ideal candidate will be committed to providing high-quality therapy services in a home setting and will thrive in a supportive environment focused on patient care. This position involves traveling to patients' homes and providing personalized physical therapy.
Key Responsibilities:
- Conduct comprehensive assessments and develop individualized treatment plans for patients in their homes.
- Implement therapeutic exercises and interventions to improve patients' mobility, strength, and overall function.
- Educate patients and their families on exercises, techniques, and home care strategies to support recovery.
- Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
- Maintain accurate and thorough documentation of patient care activities in compliance with regulatory standards.
- Collaborate with interdisciplinary teams to coordinate care and meet patients’ needs.
- Travel to patients’ homes within the designated service area; mileage will be reimbursed at the government rate.
Qualifications:
- Current state licensure as a Physical Therapist in Oregan.
- Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
- Previous experience in home health preferred but not required.
- Strong communication, organizational, and interpersonal skills.
- Ability to work independently while maintaining a focus on high-quality patient care.
