Job Description:
EliteCare Medical Staffing is looking for a dedicated and experienced RN-Care Manager to join our Home Health team in Clearlake. In this role, you will be responsible for managing patient care in a home health setting, ensuring that patients receive high-quality care tailored to their individual needs.
Responsibilities:
- Coordinate and manage patient care plans in home health, providing comprehensive support to patients and their families.
- Conduct thorough assessments and evaluations of patients’ health status and needs.
- Collaborate with healthcare professionals to develop and implement effective care strategies.
- Educate patients and their families about healthcare management and available resources.
- Ensure compliance with all relevant regulations and standards for home health care.
- Travel to patients’ homes as required; mileage reimbursement will be provided at the government rate.
Qualifications:
- Current Registered Nurse (RN) license in the state of California.
- Home health experience is preferred.
- Must possess a clean driving record.
